Membership Details

  • Annual Membership Fee:
    • Adults: $299 per person
    • Children (under 18): $199 per child
  • Payment & Renewal:
    • The full membership fee is due upon enrollment.
    • Membership begins immediately and is valid for 12 months.
    • Renewal occurs every 12 months.
  • Service Payment:
    • Payments are due at the time of service.
  • Maximizing Benefits:
    • Members must schedule and complete all covered appointments within the 12-month period.
    • Missed appointments will not be refunded.
